18. When issuing a pulmonary health certificate or a notice of referral, the physician shall provide the original to the employer and shall send a copy to the worker and to the director of public health. A copy of the notice of referral shall be sent to the worker’s accredited association.
That certificate and that notice shall be kept in the worker’s medical record in accordance with paragraph 5 of section 127 of the Act respecting occupational health and safety (chapter S-2.1).
